Maybe it is the Immunization Record Card (yellow paper card). It is issued by your paediatrician.
The card list (recorded by the nusre but not mom) all the immuniations (with date) received by your sons. Also, you need to provide this card to school to prove the kids have all the required immunization before they go to school.
After the immuniation, they will update the card for you.
As I know it is free to get the first card from my doctor. It will be charged if we request the replaced one.
they a not walk in clinic, is specialist for children...
i just called them this morning , the nurse say : "ok...remind me in your next visit & we shall issue one for them...remember to copy us their history..."
actually, i hv already passed them all my children's HK record for their reference & that's why the doctor suggested us to take some more vaccine...
anyway, good that now i know how to follow up......

I didn't get this card from my family doctor. Perhaps I should check with my doctor as well.
