My maid is always absent mind even came to HK around 1 year. She decided to go back home and informed me 2 days ago. Do I need to ask her to sign any letter ? any sample ?

what should include in the letter .. eg name of both employee and employer / contact number / last date / signature and the date. anything else ?

Is this in Indonesia ? Do I need to ask her to give one in English ?

should include :
1/ Date
2/ effetive date (i.e. one month after date of resignation)
3/ To who (name of employer)
4/ from who (full name of employee)
5/ Contract ref/no.
If she doesn't know English, ask her to have the agent or fd to translate and write on the same page.
